Cambridge Examination Results Transcript Request
The transcript request form can be found on the SBHS website at:

Read through the instructions on the request form carefully.
Important points:
• There is no charge for this service from Cambridge.
• A candidate may only apply for a results transcript once they have been accepted by a
university.
• Fill out the form in UPPERCASE LETTERS only.
• You must include a copy of an official ID (driver’s license, passport, or birth certificate).
• Under Examination Series: Enter each year you took an AICE exam separately. Exams series are
either June or November, plus the year they were taken - i.e. June 2018
*If you are taking any exams this spring, you can list June 2020 on the form and Cambridge
will send the results to the university or college when scores are released in August.
• Under Qualification: Enter AS & A Level for exams and AICE Diploma if you have earned the
diploma.
• Enter our centre number (UX014) and your candidate number.
• Under Recipient Details: Enter the name of the person receiving the results transcript or enter
“Admissions Office” if you don’t have an individual name.
• Email application to [email protected] with “Results Transcript Application” in
the subject line.
